The correct answer is (C).

Correct parallel structure requires all of the listed phrases ("Avoiding fatty foods", "exercising regularly", "visiting the doctor") to begin with -ing verb forms as in choices (C)and (E).

In addition, the compound subject requires a plural verb ("were recommended", not "was recommended") which is found only in choice (C).
